
Humana and PatientPing Partner to Improve Care Coordination
The partnership could improve patient outcomes and reduce cost of care.

With the partnership, Humana will have access to PatientPing’s provider enablement platform to monitor acute and post-acute events in real-time. The real-time notifications will activate the member’s care team who use the platform and will provide information to point of care providers, including indicators to help coordinate transitions.
Humana aims to reduce emergency department visits, preventable admissions and readmissions and improve connectivity to community-based providers after discharge.
“Our mission is to connect providers to seamlessly coordinate patient care,” said Jay Desai, CEO of PatientPing. “The regional infrastructure enables providers at health systems and health plans to collaborate with the goal of improving quality and reducing total cost of care,” he added.
And Humana is not the only health insurer partnering with a digital health solution company to improve patient outcomes and lower the cost of care.
