PatientPing and Humana today announced a partnership to bring a new care coordination focus to Humana Medicare Advantage members in North Carolina.
With the partnership, Humana will have access to PatientPing’s provider enablement platform to monitor acute and post-acute events in real-time. The real-time notifications will activate the member’s care team who use the platform and will provide information to point of care providers, including indicators to help coordinate transitions.
Humana aims to reduce emergency department visits, preventable admissions and readmissions and improve connectivity to community-based providers after discharge.
“Our mission is to connect providers to seamlessly coordinate patient care,” said Jay Desai, CEO of PatientPing. “The regional infrastructure enables providers at health systems and health plans to collaborate with the goal of improving quality and reducing total cost of care,” he added.
And Humana is not the only health insurer partnering with a digital health solution company to improve patient outcomes and lower the cost of care.
In January, Aetna announced it was teaming up with Apple to release a new mobile health app called Attain, which will personalize digital health regimens for beneficiaries.
Last year, Fitbit linked up with Blue Cross Blue Shield to help the health insurer’s members create and maintain healthy habits. The partnership also provided discounts for Fitbit devices and a bundle that includes a gym membership for those who use the insurer.
These moves from big insurers to work with digital health companies further reinforces the notion that providers want their members to receive better, cheaper care.
