Hackensack Meridian Health partnered with NowPow to help improve patients’ health after they leave the care of the hospital.
The partnership aims to fill care gaps and overcome obstacles the population is facing.
NowPow is a digital platform that provides patients with personalized referrals to community resources. Its personalized platform connects patients with food banks, rental assistance, transportation assistance to behavioral health, and caregiver and mental health support.
“It is essential for health networks to take a holistic approach to their patients’ health and we are proud to partner with NowPow to help address social needs impacting our communities,” Robert C. Garrett, FACHE, chief executive officer of Hackensack Meridian Health, said. “Our mission is to transform healthcare and part of this mission is creating innovative ways to bridge health equity gaps and ensure that all patients achieve their best health.”
The health network is using NowPow’s suite of tools, including PowRx, NowRx, and FindRx to support patient needs. The tools create personalized referrals to high quality, verified services in more than 100 languages and can be shared digitally through email or text message, or print.
PowRx is integrated with Epic and allows Hackensack Meridian Health’s care team to refer patients to community resources, connect referrals with the patient’s medical record, and receive confirmation once such services are delivered. NowRx allows the health network’s team members to connect patients and families with community resources without using personally identifiable information. With FindRx, patients get access to NowPow’s referral database to find resources they need and share referrals with others.
